https://motube.smithandtech.com/videos/watch/12ac22a3-8924-4ef1-af6b-e1dcc3b424bb Jennifer Homendy, the chair of the National Transportation Safety Board (NTSB), said Tuesday that multiple failures at institutional and individual levels were likely to blame for Sunday night’s deadly collision between a landing Air Canada jet and a fire truck on the runway at New York City’s LaGuardia Airport.   Ms. Homendy said there had been conflicting reports as to who was responsible for controlling ground movements at LaGuardia that night and that the fire truck lacked transponder equipment modern air traffic control uses to monitor vehicle movements.   She cautioned against reports that placed blame on controller distraction, saying that one person could not be responsible for such a catastrophe in modern air travel.   “We rarely, if ever, investigate a major accident where it was one failure,” she said. “There are multiple, multiple layers of defense built in to prevent an accident.
